What is Good to be Home?

Good to be Home is a restorative model of client support delivered at home.

It aims to provide choice, promote and allow people to live their life to the full, whatever their health needs.

We promote dignity by establishing an outcome focused partnership between clients, support workers and other health professionals.

Support is tailored to the individual’s needs and goals, and family / whanau and friends are involved in planning if so desired.

Support is proactive and co-ordinated and brings together the skills of a range of carers and health professionals.

Good to be Home has been developed through research and evidence based practise.

Good to be Home Services:

  • Goal setting that recognises social as well as physical needs, such as participating in regular activities, and catching up with family and friends
  • Activities to help clients maintain or regain skills, mobility and confidence
  • Support with personal care, medication and household management
  • Specialist nursing services such as wound care
  • Coordinated access to a range of support workers and health professionals such as nurses, physiotherapists and occupational therapists

Our Philosophy for Good to be Home

  • The client is at the centre of the decision making process and has control and choice
  • Their situation is assessed holistically, to take account of cultural, social, physical and emotional needs
  • If so desired, family / Whanau and friends are involved in the care planning and goal setting process
  • Services are coordinated to improve access and overcome duplication
  • The aim is for the client to enjoy life and remain in their home for longer

The Good to be Home funded service may be accessed through your local needs assessment agency (NASC), by referral from your GP, or following discharge from hospital.
