Explore

Explore offers new possibilities to develop the potential of people with disabilities as well as the potential of support workers. The specialist team works with the staff of providers of disability support services, to increase the knowledge and skills that enable people to fulfil their potential and live quality lives in their community.

The Explore transdisciplinary team has expertise in behaviour support, clinical psychology, nurse consultancy, dual diagnosis (mental health), occupational therapy, physiotherapy and speech language therapy

The team provide

  • Assessment and development of support plans and interventions.
  • Coaching and development of staff expertise.
  • Promoting and facilitation direct access to health care services in the community.
  • Relationship development skills.

Medically Fragile Children's Service

This service, currently available in Waikato, strives to promote quality family life, community participation and maximum independence for medically fragile children aged up to 14-years and their families/whanau.

It involves short-term, home-based support and respite services which allow families to achieve a balanced life while incorporating their medically fragile child's health needs.

Medically fragile children have health needs which require a registered nurse overnight. Our service focuses on children who have been discharged from the neonatal-unit and return home with complex needs. Support workers, under the supervision and guidance of a registered nurse, assist the family to cope on a daily basis by providing:

  • oxygen therapy
  • tube feeding
  • suctioning
  • household management
  • overnight support.

We offer a planned approach from hospital to home to reduce the number of crisis episodes and minimise family/whanau's stress levels. Our goal is to increase their confidence in caring for their medically fragile child and to enable the child to enrich its own growth and development.

We support families to care for their child at home, in the community. We aim to reduce their feelings of isolation by building trusting, respectful relationships and providing a quality service.
