Support Worker – Mental Health & Addiction Services

The Community Support Worker (Facilitating Recovery) – Mental Health & Addiction Services assists people experiencing mental illness or addiction problems to live independent and successful lives in the community.

Focusing on the recovery and strengths of the people we support, this role will work as part of a team that includes:

  • Mental Health & Addiction Services Managers
  • Mental Health & Addiction Services Community Support Leader
  • Mental Health & Addiction Services Administration
  • District Health Board Mental Health Service Key Workers/ Case Managers
  • other community support workers.

Principle accountabilities may include:

  • Participating in the client's initial assessments and subsequent reviews.
  • Involving the client and their family/whānau in developing recovery plans, goals and objectives.
  • Work in partnership with clinical services to ensure recovery goals are met.
  • Maintain professional relationships with Community Mental Health Team Key Workers and Case Managers.
  • Work in partnership with agencies such as Work and Income New Zealand and Child, Youth & Family, to ensure that client needs are met.
