Enabling people to actively manage their long-term health condition
Our Te Whiringa Ora service is about empowering Kiwis with long-term conditions to enjoy a higher quality of life and greater wellbeing, by:
- extending the vital role of general practice and hospital care into the home
- supporting people to access the right service at the right time
- enabling people to better understand and manage their condition.
This innovative service seeks to ease the pressure on the New Zealand health system by taking health integration to a new level.
We work in partnership with a diverse range of local health and social services to connect each community’s existing health resources and to reduce the number of unplanned hospital admissions.
Together, we actively promote the holistic values of Whānau Ora by supporting all aspects of an individual’s wellbeing and by sharing information through the use of an electronic patient record.
Our skilled case managers (who are registered nurses) work alongside the person involved and their family/whānau to complete a thorough assessment, develop a holistic care plan and to ensure coordinated access to the services they need.
Additional one-on-one support, education and guidance is provided by our specially trained kaitautoko (health coordinators), enabling people to actively manage their condition well beyond our direct involvement.
