NASCs

If you are involved in needs assessment and resource allocation, our professional local teams will work with you to ensure that the people you refer to our services receive the high-quality support they need in their own homes and communities.

With more than 20 years' experience, our dedicated team has the knowledge, systems and expertise needed to provide people with personalised support, regardless of their age or any injury, illness or disability they may have.

Household management and personal care

Our professional HealthCare NZ team provides high-quality home help and personal support services nationwide and is committed to enabling people to remain in their own homes and to enjoy greater wellbeing and quality of life.

Depending on each person’s individual needs, we can support them with:

  • their everyday tasks such as cleaning, vacuuming, washing, grocery shopping, meal preparation and dishes
  • any personal support needs that they may have, including grooming, dressing, bathing, showering, mobility and toileting.
Community nursing and governance

Our professional HealthCare NZ nursing team has the clinical skills, knowledge and experience required to provide people with either the basic or complex healthcare they need in the comfort of their own homes.

Our specialist community nursing services include:

  • wound and ulcer management
  • intravenous therapy
  • continence care
  • PEG feeding
  • stoma therapy
  • medication management and oversight
  • methadone support
  • social rehabilitation assessments (ACC)
  • diabetes management
  • cardiopulmonary conditions management
  • services for medically fragile children
  • palliative care.
Mental health and addiction support

Our professional HealthCare NZ Mental Health and Addiction team specialises in providing people with the holistic support they need to reach their personal goals, and to overcome any mental health and/or addiction issues they may be facing.

Depending on each person's individual needs, this may involve supporting them to: 

  • find practical ways to deal with life’s challenges
  • maintain their medication regime
  • regain their resilience
  • build stronger life management skills
  • maintain affordable accommodation
  • improve their confidence and self-esteem
  • work towards their goals
  • build stronger social networks and connections
  • become more involved in the community.
Disability support

At NZCare, we are committed to supporting people with disabilities to make their own choices, to do the things they enjoy and to live with more independence.

We provide high-quality specialist support for people of all ages, including those with  intellectual disabilities,  physical disabilities, autism spectrum disorders or Huntington’s Disease.

To meet each person’s individual needs, we work alongside them and their family/whānau each step of the way, providing personalised support that may include:

  • supported living options that empower people to live full and independent lives, either in their own homes or in one of our residential houses
  • vocational programmes that support people to learn new skills, to do the things they enjoy and to be a part of their local community
  • flexible respite support, including in-home, out-of-home and host-family options that enable people to take a safe, enjoyable and supported break away from their usual home environment or support system
  • individualised funding options that enable people to take maximum control over the support services either they or their family member receives.
Specialist disability support

At  Explore, we specialise in creating disability support solutions that promote long-lasting benefits for people of all ages, including those with intellectual disabilities, and with high or complex needs.

Our professional team works in a transdisciplinary way, bringing together expertise in clinical psychology, behaviour support, speech and language therapy, physiotherapy, occupational therapy and nurse consultancy, and supports people to:

  • communicate their wishes and needs
  • work through challenging behaviour
  • overcome any swallowing difficulties
  • learn new skills and strategies
  • make a transition from one stage in their lives to another.

We also provide high-quality workforce training that enhances the specialist skills, knowledge and capability of service providers and their employees.

Early supported discharge service

At HealthCare NZ, we can provide people with the specialist nursing and home support they need to make a safe and early transition home from hospital.

Our nurse-led teams have the skills, knowledge and experience needed to provide health services that enhance each person’s recovery, wellbeing and independence.

In Christchurch, our CREST teams are supporting the DHB to free up scarce hospital beds by providing an early supported discharge service that effectively manages people’s health needs in their own homes.

Long-term condition management

With chronic conditions accounting for 80% of preventable deaths in New Zealand, we are committed to empowering people with long-term health issues to enjoy greater wellbeing and a higher quality of life.

Throughout the country, our professional nursing teams provide specialist home-based care to people with chronic conditions such as diabetes, heart and lung disease.

And in the Eastern Bay of Plenty, our innovative Te Whiringa Ora service is enabling people to achieve greater long-term independence and wellbeing by:

  • connecting the community’s existing health resources
  • extending the vital role of general practice and hospital care into the home
  • giving people the tools and knowledge they need to manage their long-term condition beyond our direct involvement.
Personal medical alarms

Our Freedom Medical Alarms enable people to live safely and independently in their own homes by ensuring that expert medical attention is just the push of a button away.

Worn as either a waterproof pendant or watch, our alarms give people a direct link to an emergency ambulance service that is monitored at all times, by a trained and experienced emergency team.

Freedom Medical Alarms is a Work and Income NZ accredited personal medical alarms provider and funding is often available for our service. To find out more, visit www.freedomalarms.co.nz.
