Healthcare NZ Community Health provides home and community-based support services and nursing services. For more than 25 years, we have been supporting people to live their lives the way they want to, in the home of their choice, and in their communities. Our aim is to support people to live as independently as possible, for as long as possible.

Our services are free for eligible residents. Privately funded care is also available.

In-home support

There’s no place quite like home, where you are close to your family and friends, favourite activities, routines and community. If you or someone you love needs support to remain living at home, we can offer a full range of services.

Support services include:

  • personal care: grooming, dressing, bathing, and mobility
  • domestic support: help with everyday tasks such as cleaning, vacuuming, washing, meal preparation and dishes
  • support to enable you to develop greater strength, mobility and confidence
  • a range of nursing services that meet your short term or ongoing medical needs.

Good to be home

Our goal-based Good to be home programme works with you to achieve your health goals. Our professional Good to be home team works alongside you and your family and whānau each step of the way.

As a restorative service, Good to be home is designed to enhance your quality of life and independence by supporting you to do everyday tasks for yourself.

Examples of activities or daily needs you may need support with include:

  • washing, dressing, grooming and toileting
  • preparing and managing meals
  • taking medication safely
  • exercises that will increase your mobility, strength and coordination
  • assistance to achieve mobility goals such as walking confidently to the park
  • learning new skills to manage symptoms of any illness you may have, such as breathing exercises for people with respiratory problems
  • keeping socially connected, such as being part of a community group, club or activity programme.

Long term condition support

If you have a long term condition such as diabetes or heart disease, Healthcare NZ Community Health can provide you with the support you need to enjoy maximum independence and quality of life.

Our professional team will work with you and your family/whānau (if you wish) to create a personalised support plan that meets your needs.

We can offer a full range of support including:

  • personal care: grooming, dressing, bathing, and mobility
  • domestic support: help with everyday tasks such as cleaning, vacuuming, washing, meal preparation and dishes
  • specialist nursing oversight which provides ongoing assessment and treatment of your medical needs.

And because we are committed to enabling people to lead happier, healthier lives, we are continuing to develop innovative new services for people with long-term health conditions.

Nursing services

Whether your needs are short or long-term, Healthcare NZ Community Health can provide a full range of specialised nursing services in your own home.

We’ll partner with you to help you maintain independence and quality of life within your home and community. Our nursing team offers care and support that is flexible and responsive to your needs, backed by an on-call nurse available 24 hours a day, 7 days a week.

Our extensive services include:

  • comprehensive nursing assessments
  • early supported discharge from hospital
  • wound care management expertise
  • medication support and oversight
  • diabetes services – monitoring and insulin support
  • cardiopulmonary support – assessment, chronic care and management of pleural drains
  • cardiothoracic support – post-surgery, allowing early discharge
  • continence management – short and long term
  • ongoing support for older people presenting with multiple health conditions
  • falls prevention programme – delivered by trained Physiotherapists and Registered Nurse
  • palliative care - working alongside GPs and palliative care services 
  • health education to promote wellbeing and maximise independence
  • clinical review of care plans to ensure we are responding to changing health needs
  • maintaining professional relationships with GPs, Practice Nurses and specialist health services to ensure we are working together to meet the goals of the people we support.

Reintegration services

Our Te Aranga Mai reintegration programme supports ex-offenders to turn their lives around by identifying their needs and supporting them to access appropriate services. 

We work in partnership with the Department of Corrections to support each person's wellbeing, their journey to reintegration and their personal ambitions and goals. We are committed to supporting our clients to have greater independence and quality of life. 

The expertise in our team means that we can tailor our services to each client and their family/whanau where appropriate.

Individualised funding

Individualised funding (IF) is a way of paying for disability support services that allows you greater independence and flexibility with your allocated hours.

IF empowers people with disabilities to personally manage the hours they have been allocated for household management and personal care. Managing your own support can range from choosing your support workers and planning how you want to use the hours of support, to employing support workers and managing all aspects of your service delivery.

IF must be managed through a contracted IF host organisation like HCNZ Community Health.

  • support for medically fragile children (Hawke's Bay and Waikato)
  • in-home dialysis (Southern)
  • wellness checks (Southern)
  • palliative care (Southern, Canterbury, Hawkes Bay, Wellington, Hutt Valley/Wairarapa).
