What are long-term and chronic conditions?

As the name suggests, a chronic or long-term condition is an illness or medical condition that continues for a long time or for the rest of your life. Some of the most common chronic conditions in Aotearoa NZ include:

Respiratory conditions

Asthma, COPD, pulmonary hypertension and more; these affect the lungs and airways.

Cardiovascular diseases

Heart failure, arrhythmia, CAD, stroke, congenital heart disease; these conditions affect the heart and circulatory system.

Diabetes

A condition in which the body does not produce enough insulin, which causes high blood-sugar levels, and can lead to other complications.

Chronic pain

Pain that lasts beyond a usual recovery period or beyond three months, affecting daily life.

Autoimmune diseases

Multiple sclerosis, lupus, celiac disease, rheumatoid arthritis and more; these occur when the immune system malfunctions and attacks the body’s own cells.

Long-term conditions are common amongst New Zealanders and they are rising in prevalence. There are inequities in the distribution of these conditions, too: Māori and Pacific peoples are more likely to suffer from them.

According to Health NZ | Te Whatu Ora, one in four New Zealanders suffer from multiple chronic health conditions. Not all of these people, however, will require chronic illness support such as long-term pain management, home help or at-home health care services. Each case is unique—some conditions can be self-managed; others pose significant challenges in daily life.

If your chronic illness or long-term condition is affecting your quality of life, it may be time to seek extra support. Get in touch with the HealthCare NZ team and let’s discuss what you need.

Here are a few of the most in-demand HealthCare NZ services we provide to people with chronic care needs. If there is a specific need not covered here, please get in touch with the team and ask.

Home help

A chronic illness or chronic pain often throws up barriers when it comes to completing everyday tasks. Our carers can remove these barriers by helping with cooking, cleaning, dressing and other jobs.

Medication management

Pain management support and administering medications is often an important part of long-term chronic care.

Personal care

HealthCare NZ team members are experts in providing personal care that’s professional, competent and respectful. Hygiene, toileting and other personal-care tasks can be more difficult to self-manage without chronic pain support or help with your long-term illness.

Our at-home healthcare and support services are designed to support the whole person, enabling and empowering better quality of life and helping you achieve your long-term goals for acute and chronic pain management, removing the obstacles that health issues often create.

Frequently answered questions: Long-term care

How are chronic conditions diagnosed?

Doctors and medical professionals can diagnose long-term conditions. A diagnosis is the first piece of the puzzle; from there, you can be referred for further services. We recommend that if you are experiencing long-term chronic pain or symptoms, you seek medical advice and treatment options first.

What causes long-term or chronic illnesses?

The causes of chronic conditions vary widely—and for some, there are no definitive answers. Some are passed on genetically, some are related to lifestyle factors, and others may occur as the result of a previous injury, event or illness.

How are chronic conditions treated?

The treatments for long-term conditions are as varied as the causes. Even individuals with the same diagnosis will require different treatment plans and symptom management. That’s why it’s our policy at HealthCare NZ to tailor our care to your unique needs and challenges.

How can I maintain a good quality of life while living with chronic pain or illness?

Again, there is no single answer to this! A lot of managing long-term conditions is about responding to your unique symptoms, finding treatments and medications that work, and getting the right support to lower barriers in everyday life. A comprehensive, chronic-illness support or chronic-pain care plan will respond to changes and take into account the wellbeing of the person as a whole.
